- Identity Card (MyKad) – Please provide a crossed copy marked "For UTP Use Only".
- SPM result or equivalent qualification (e.g., O-Level, IGCSE).
- Parent's or guardian's Identity Card (MyKad).
- Recent passport-sized photograph (blue background).
Applicants are advised to ensure that they meet all programme entry requirements and submit all required supporting documents when completing the online application.
Please note that applications will be assessed based on the completeness of the submitted documents and compliance with the programme's entry requirements. Applications with incomplete documentation or those that do not meet the minimum entry requirements may not be considered for admission.
Applicants who are unable to provide the required documents at the time of application are encouraged to apply for a subsequent intake once all supporting documents are available.
