Complete the application form and rename the file as your Passport no with your nationality. Please complete the application form using Acrobat Reader 8.0 and do not scan the form.

e.g B123456-Indonesian (non-Malaysian).
  • You will need to include certified documents to support your application i.e academic transcripts. Where appropriate please provide official certified English translations. Copies of academic transcripts, statements, results etc that have been verified by appropriate personnel (original issuer, school principal, judge etc) as genuine reproductions.
